Must be four (4) on or before September 1st.

*Copy of Certified Birth Certificate
* Proof of Ga. Residency (Miller County)
Examples: Utility bill – such as:
Light or water, Current lease or property tax notice,
Homeowner’s insurance bill, Vehicle registration / Tag purchase receipt,
Mortgage statement, Home Phone bill …no cell phone bill
* Social Security Card
* Health Insurance Card
* Immunization Form 3231
*Form 3300: Ear, Eye, Dental and Nutritional Screening
*Health Dept. will be on site on Thursday (2-23) only.

The Georgia Statewide Longitudinal Data System (SLDS) is a free online tool developed by the Georgia Department of Education that helps States, Districts, Schools, and Teachers make data driven decisions to improve student learning. Parents now have access to their children’s SLDS data through the SLDS Parent Portal. At Miller County Elementary, we believe that children start their future when they are young. Therefore, we provide an education to students in grades K-5 that is full of variety. However, we do not neglect the core subjects.
